Another point may be that the headphone volume should be low as the higher the volume the lower the pitch we hear for the same frequency. A very good book to read is Sound and Hearing - Life encyclopedia - Stevens, Warshofsky. Phil Ramone in his book Making Records mentions that if a singer is out of tune first thing to correct is the headphone mix.
